Registration forms for enrolled students
Please download and complete the following forms and submit hard copies by May 31, 2017 to complete your enrollment.

- General Guidelines & Rules of Conduct Form (Submit page 1 only, after carefully reviewing entire document)
- Immunization Requirement & Physical Exam Form (2 pages)
- Medical, Emergency, and Health Insurance Information and Authorization Form (4 pages)
- Liability Release & Waiver (2 pages)
- Electronic Devices Policy (1 page)
- Publicity Grant and Release (1 page)

Even if you are a returning student, please submit ALL of the required forms for 2017, IN THEIR ENTIRETY, with updated information. This is especially important regarding the Medical Form.

Workshop Tuition: $2,280
Room with meal plan: $1,800 (All residential students will be placed in a double room and must be on the meal plan)
Meal plan for Non-Residential Students: $725 (lunch and dinner only)
Tuition may be paid in full upon program acceptance, or incrementally as per the schedule below. An $85 fee applies to all late payments. Tuition payments can be made by check or credit card. Please note that a service charge will be applied to all credit card payments.
1st Payment: Seven days after acceptance notice: $1000 Non-refundable registration deposit. Pay your deposit HERE by credit card.
2nd Payment: May 1, 2016: $1280. Pay your 2nd payment HERE by credit card.
3rd Payment: June 1, 2016: Remaining Balance. Pay HERE for housing students. Pay HERE for non-housing students (lunch/dinner meal plan).
Full Tuition Payment: Pay your entire tuition + room/board payment as a lump sum HERE by credit card.
