Registration forms for enrolled students
Please download and complete the following forms and submit hard copies by May 31, 2017 to complete your enrollment.

- Immunization Requirement & Physical Exam Form (2 pages)
- Medical, Emergency, and Health Insurance Information and Authorization Form (4 pages)

- Electronic Devices Policy (1 page)
- Publicity Grant and Release (1 page)

Students will receive their workshop schedule
Students interested in Auditioning to perform for a guest artist master class must sign-up for an audition slot at registration
(YAP students do not need to audition they will be performing in the appropriate master class)
4:00pm-4:30pm Parent Orientation (optional-meet at Lawrence House)
Meet our faculty and staff, ask questions, and brief tour! *This is optional
4:30pm Farewell to Parents
5:00pm Student Orientation/Dinner

Parents, please plan to help pack, pick up luggage, return OneCards and keys and request final room inspection before 10:00am
10:00am-12:30pm Students will have their final practice, rehearsal, and sound check in Sage Hall/Parents are welcome to enjoy the Smith College Campus and Northampton town
12:30pm Doors open for Final Student Gala Concert
1:00pm-3:00pm Final Student Gala Concert in Earle Recital Hall | Sage Hall | Smith College

How to reach your child at Lyra:
In the event of an emergency that necessitates immediate contact with your child, please call Rachel Odo, Executive Director, at 917-705-5089. **This is for emergency contact only.
For all other matters, please email email@example.com and your message will be relayed.
For urgent matters that cannot be handled by email, please call Rachel Odo between 9:30am-10:30am and she will respond as quickly as possible.
As with almost all residential summer programs, cell phones and other electronic devices are not permitted during the Workshop. There will be one “dorm phone" available for students to make brief personal calls most evenings. This phone will be shared among all students and available for approximately 1 hour. Please be mindful that other students may be waiting for a turn and keep calls short.
Workshop Tuition: $2,280
Room with meal plan: $1,800 (All residential students will be placed in a double room and must be on the meal plan)
Meal plan for Non-Residential Students: $725 (lunch and dinner only)
Tuition may be paid in full upon program acceptance, or incrementally as per the schedule below. An $85 fee applies to all late payments. Tuition payments can be made by check or credit card. Please note that a service charge will be applied to all credit card payments.
1st Payment: Seven days after acceptance notice: $1000 Non-refundable registration deposit. Pay your deposit HERE by credit card.
2nd Payment: May 1, 2016: $1280. Pay your 2nd payment HERE by credit card.
3rd Payment: June 1, 2016: Remaining Balance. Pay HERE for housing students. Pay HERE for non-housing students (lunch/dinner meal plan).
Full Tuition Payment: Pay your entire tuition + room/board payment as a lump sum HERE by credit card.
